Illinois Birth Certificates 

According to the Illinois Vital Records Act, the local registrar may provide a certified copy of birth record to a parent listed on the birth certificate or a legal guardian presenting evidence of guardianship. A legible copy of the parent or guardian’s valid/legal photo identification card must be presented/submitted. If unreadable or expired, the request will not be processed.

• Legible copy of the parents or guardian’s valid/legal photo ID
• If you were born before 1985, you must contact the Jackson County Clerk, 618-687-7360
• Cash, credit card or check accepted
• $18 for the first certificate and $5 for each additional copy
