Jackson County Health Department provides case management for all Jackson County pregnant women and infants who receive an Illinois medical card.

What is case management?

      The goal of Family Case Management is to:

      1) Decrease infant mortality and morbidity,

      2) Improve pregnancy outcomes and

      3) reduce incidence of prematurity and low birth weight.

  • Case managers provide education, safety information, and assistance with accessing community resources.

Jackson County Health Department also provides all the immunizations recommended for children.
