Health Education
The Health Education Division focuses on several community health priorities. Much of the programming is driven through collaboration with individuals and local organizations. All staff actively participate with the Healthy Southern IL Delta Network, Jackson County Healthy Communities Coalition, and numerous other local action groups. Funding for area efforts comes from (state government, local tax dollars, non-governmental organizations, universities, non-profit hospitals, and other targeted grants). Health Education also is responsible for conducting a community needs assessment and county health improvement plan every 5-years. Below are the key programs that are offered:
Chronic Disease Management
Chronic Disease Self-Management and Diabetes Self-Management classes are available. These are free-to-you programs that last 6-weeks and help individuals with chronic health conditions. Contact jessicaf@jchdonline.org
Community Health Improvement Plan (IPLAN)
Jackson County Health Department is responsible for conducting a county health needs assessment, and developing a health improvement plan every 5 years. Currently focusing on 1) Addressing Behavioral Health including mental health and substance abuse, 2) Community-wide Health Equity and Chronic Disease Reduction, 3) Improving Family and Community Engagement-Impacting Social Determinants of Health. Nutritional and Physical Activity

School Health
