Birth Certificates - According to the Illinois Vital Records Act, the local registrar may provide a certified copy of birth record to a parent listed on the birth certificate or a legal guardian presenting evidence of guardianship. A legible copy of the parent or guardian's valid/legal photo identification card must be presented/submitted. If unreadable or expired, the request will not be processed.

Death Certificates - According to the Illinois Vital Records Act, the local registrar may provide a certified copy of death record to a duly authorized agent (includes heirs, children, spouse, administrators, executor, attorney for estate) having a personal or property right/interest in the death.
